What can I expect at my appointment?   

Your appointment will begin with a review of the Medical Record Forms you completed, a case history,  any x-ray reports, cat scan, and MRI imaging you bring or have faxed to the office, the onset, date or day of your symptoms began or injury or fall occurred and level of pain or discomfort you are experiencing .  A through chiropractic, orthopedic, and neurological examination will be completed as part of your assessment. Treatment options will be discussed based on my findings and a plan of care will be initiated. 


Insurance Policy of D'Amato Chiropractic

Your insurance is your group plan or chosen policy.  We do not call your insurance company to find out your benefits, if you want to know your benefits, it is up to you to call  MEMBER SERVICES on the back of your card, or go online to see what your chiropractic coverage is and ask if Dr. Adrian D'Amato is on your plan.

     *Ask if your plan has chiropractice benefits.
     *Ask how many chiropractic visits your coverage has for a year. 
     *Ask what your COPAY is for chiropractic.

Patient is responsible to pay a COPAY in the office, each day services are performed. D'Amato Chiropractic Team will bill your insurance claim.                  Once the payment is received, if there is a balance, a statement will be mailed to you. Parent of a minor will receive a statement for the child.


No Insurance No Problem

Times have changed and patients who have insurance, but have a high deductible or high copay, are choosing to pay cash versus have their insurance billed.  Or, you just don't have any insurance. Either way, the office has available a Time of Service Fee Schedule, ask at the front desk for additional information.  If you choose this, payment is expected each treatment date, no running up a bill to be paid at a later date.


Cancellation Policy - 24 Hour Notice Appreciated

Please call the office ae early as possible to reschedule. If we are not in the office, leave your name, phone number and short message.

The voicemail is available 24/7. 

NO SHOW  will be assessed $40 no show fee.
